Patients are required to be seen annually for well child checks to remain in compliance with practice guidelines. Well child checks are the ideal time to ask developmental and general health information questions. It is also a time to obtain new health information or simply review basic preventative health information as a part of being proactive in caring for your child’s health. Adolescents also have an opportunity to begin learning to “find their voice” and to begin speaking up, asking questions, and taking ownership of their health choices. School and athletic forms may be filled out during this visit at no additional charge. Parents are encouraged to bring the patient’s immunization card to be updated at these visits. Patients ages 6 months to five years old (60 months) will need to complete the Ages and Stages Questionnaire (ASQ) for the Well Child Check. The questionnaires are only required for ages 6 months, 12 months, 18 months, 24 months, 36 months, 48 months and 60 months. Just click on one of the links below and bring the complete d ASQ to your child’s appointment. You do not have to score the ASQ.
Medication checks are required for patients who take medications on a continuous basis. Med checks should be scheduled every 1, 3 or 6 months depending on the type of medication and the recommendations of the physician. Failing to schedule these appointments regularly may delay refills.
Patients are required to be seen for Asthma Reviews every 3, 6 or 12 months depending on the severity of their asthma and the recommendations of the physician. These visits are used to check peak flows, refill medications, check devices, review technique or train on new devices, and develop a current asthma action plan. The patients may also choose to participate in our Asthma Group Medical Visit. For more information, look on the right side of the page and click on the Group Medical Visits link. |
