OUR METHODS Nurse Advice Line - For nurse advice questions, you can call our main number at 919-235-0543 and select the nurse advice line, during normal business hours. Please leave your child’s name, date of birth, a brief description of your question, and a return phone number. Every effort is made to return all calls by the end of the day. However, non urgent calls received after 2pm may not be returned until the next business day. Refill Request Line - For medication refill requests you can call our main number at 919-235-0543 and select the pharmacy refill request line or by selecting the link on our homepage and sending an email. Please allow 2 to 3 business days for routine refills. If you are a pharmacy requesting refill authorization, you can fax a request to 919-235-0542 or leave a voice message. If you are a patient requesting a refill, please ask your pharmacy to call us for authorization or leave a detailed message. Controlled substances such as Concerta, Ritalin, Dexedrine, and Metadate require a written prescription and cannot be phoned in to your pharmacy. **Please note: some controlled substances require frequent follow-up visits with the doctor. Controlled Substance Prescription Pickup - When picking up controlled substance prescriptions, be sure to bring your Driver’s License. You must show proof of identification in order to receive the script. Immunization Record Requests - You may request immunization records by calling our main number at 919-235-0543 and selecting medical records requests. Leave a message with the child’s full name and date of birth and a contact name and phone number. Your request will be filled within 24 hours and you will be called when the record is ready for pick up. Please note that you must pay a $5 cash fee for the immunization record at time of pickup. Referrals - It is our office policy that patients must be seen by the physician before we can initiate new referrals to a medical specialist or for any medical procedure. School/Camp/Daycare Forms - You may fax physical, day care, school, and camp forms to 919-235-0542. Be sure to include the child’s full name and date of birth and a contact name and phone number. You will be called within 5 business days when the forms are ready and must pay a $5 cash fee per form at time of pickup. Per our new office policy we can no longer fax completed forms to you or to a school/daycare. Completed forms must be picked up at our office during normal business hours and paid for at the time of pick up. Medical Records Requests - All medical copying requests must be in writing. Please allow 21 business days for processing. Download the Consent of Release - Outgoing and return it to Stepping Stones Pediatrics during normal business hours or send it by mail or fax. You will be pre-billed for the records. Meaning, once your records have been copied, you will be sent a bill to the address you write under billing information on the Medical Release Form. After you have paid the bill, the records will be released to the address you listed under Facility/Physician Info on the Medical Release Form. If you have questions about the status of your records after they have been copied, call Healthport customer service at 1-800-367-1500. |
